Understanding Cholesteatoma

A **Cholesteatoma** is a non-cancerous, but destructive, skin cyst-like growth that develops in the middle ear or mastoid bone. As it grows, it continuously sheds old skin cells and expands, eroding the surrounding bone, including the tiny hearing bones (ossicles) and even the bone separating the ear from the brain. If left untreated, it can lead to:

  • Permanent Hearing Loss
  • Vertigo (Dizziness) and Balance Issues
  • Facial Paralysis (Damage to the facial nerve)
  • Meningitis or Brain Abscess (Rare but severe complications)

Types of Mastoidectomy Procedures

Cortical (Simple) Mastoidectomy

Used for less extensive, contained infections (acute mastoiditis). It involves clearing the infected air cells without significantly changing the ear canal or middle ear structure.

Canal Wall Up (Intact Canal Wall) Mastoidectomy

The preferred method for limited disease. It removes the cholesteatoma while preserving the posterior ear canal wall, which helps maintain a more natural ear structure and often results in better hearing outcomes, sometimes requiring a second-stage procedure for hearing reconstruction.

Canal Wall Down (Modified Radical/Radical) Mastoidectomy

Reserved for very extensive, recurrent, or advanced disease. This procedure removes the posterior ear canal wall to create a single, large cavity (mastoid bowl) connecting the ear canal, middle ear, and mastoid. This ensures complete disease removal and long-term surveillance but requires lifelong ear cleaning.

Surgical Goals and Outcomes

Disease Eradication

The absolute priority is the complete removal of the cholesteatoma or infection to stop bone erosion.

Prevent Complications

Protecting the inner ear, facial nerve, and structures toward the brain from potential spread of infection.

Hearing Improvement

Where possible, the procedure is combined with Tympanoplasty and Ossiculoplasty to restore sound transmission.

Recovery and Post-Operative Care

Mastoidectomy is a major surgery requiring careful post-operative management:

  • Patients typically stay overnight in the hospital and require a few weeks off work/school.
  • A large dressing is worn over the ear for the first few days, followed by smaller dressings.
  • Strictly avoid getting water in the ear. Avoid strenuous activity and heavy lifting for 4-6 weeks.
  • Regular, long-term follow-up is essential, especially after Canal Wall Down surgery, for ear cleaning and monitoring.
